Background :
Triple negative breast cancer is a poor prognosis cancer and about 30 percent of patients may persent with brain metastasis during their disease.median overall survival is about 2 to 24 months.most of them present with neurological symptoms.
Methods:
We had 20 patients in each group and average age was 45 .these two groups received radiotherapy and then one group received liposomal doxorubicin 30mg/m2 every 28 days and other group (control) was in fallow.we did brain mri every 3 months and neurological examination monthly.some of these patients had organ metastasis like lung and liver.
Results:
In liposomal doxorubicin group we had meaningful os and PFS.OS was about 30 months and in control group it was about 16 months. neurological symptoms like epilepsy and diplopia were better in liposomal doxorubicin group and this group had better quality of life.headache was the most relevant symptom .about two thirds of patients had other organ metastasis like liver and lungs.hazard ratio in liposomal doxorubicin group was 0.7.
Conclusions :
The prognosis of patients with brain metastasis is poor and they have limited survival.because of lack of studies helping these patients we designed this study for control these types of patients.the results is acceptable and liposomal group has better survival and progression free survival.liposomal doxorubicin has better distribution in brain metastasis and less adverse effects.about 2 of patients with blindness after 2 cycles of treatment had find their vision and liposomal doxorubicin had better control on head aches .epilepsy rate decreased overall .for better evaluation we need a clinical trial which is multi centeric .
